The Government has announced six sites that will test new neighbourhood health models – half of which are led by hospital trusts.
The Department of Health and Social Care (DHSC) said that the sites – which it called ‘neighbourhood health trailblazers’ – will develop and test new models bringing together NHS services, councils and community organisations to ‘improve outcomes’, backed by £1.5m funding.
